Neito wasn't a liar. He didn't like lying. So he didn't want to confess, after all, he wasn't certain it was the truth. He didn't want to be a liar.
Feelings were confusing things. And as much as he gave A-Class shit, he didn't want to hurt their golden boy due to faux feelings.
Perhaps, the smile that Izuku gave him was worth all their future troubles.
Little did Neito know, Izuku was thinking the same about the blonde.
OR: Neito is bad at feelings and doesn't know he's in love with Izuku despite being obviously head over heels for him.
