“I suppose you could say I’m your guardian angel,” the man murmurs. His breath fogs pale between them. All of him is unnaturally warm, like Dean’s touching somebody with the sun sewn up beneath their skin. “I have known you, Dean Winchester, for a very long time.”

The righteous man is permanently damaged beyond repair, but this angel of the lord can’t afford to care as he develops a deep attachment to the childlike shell of a man he drags with him through a post apocalyptic world.
The world is in shambles, angels and demons fight on earth, monsters roam free, and humanity is doomed. Castiel only cares about keeping his human safe in the chaos.

Things go a little sideways and Sam finds himself with a child version of Dean on his hands, and a scramble to fix it. When they find the source, Sam doesn't like what he hears, but they get through it together, confronting every hard truth Dean managed to hide from Sam, and everyone else, for so long.
When Dean gets back to how he should be, the story will follow through the show, with changes based on how the events of this case would shape them.
Technically starts before the third season, inserting a case before the one in the first episode.

He had finally broken free from the prison he had been chained to by duty only to be locked into the heart of another on the pretense of chasing freedom and emancipation. “What were you thinking, Toshiyuki? What a fool”, he repeats to himself every day ever since. They will find him. He will find him. What is the point of this journey anyway if he were going to be constantly hunted down? Any minute now, any day, everything can crumble down, and he would die the most wretched death, and no one would find out, not even the loved ones he had left behind. Death itself does not bother him, or maybe it does but he does not have the time or the mental power to think about this yet. The wait is more pitiful, more agonizing. Waiting for it to happen has taken its toll on his weakened body. He might as well have died in Abashiri rather than spend weeks, months, or years waiting for his luck to run out. What luck? He knows it’s only a matter of time. He knows he is too weak and cowardly to even survive on the path he has chosen.
