Mrs. Hudson has questions. "What happened again? You said John got hurt?"
"Yes. Long story, but an escaped convict with a knife." Sherlock leaves out the words dirty and contaminated. "He got in the way." He also leaves out the fact that John, in a moment of sheer idiocy had flung himself between the knife and Sherlock's chest. "Just a clean out, some antibiotics."
There is a pause, and Sherlock can almost feel the unrest coming through the line. "You're not telling me everything." After all this time, she sees. And sees.
Of course she does, Sherlock laments. There's no one around to see Sherlock roll his eyes, but he does it anyway.

Things hadn’t felt right in 221B since John and Rosie moved back in. Everything was off. Wrong. If only Sherlock knew it was about to get even worse. But, for vegetation to return to a barren wasteland, rain from thunderstorms is necessary. A brushfire, even, to burn away the old and provide nourishment for the new… and to expose a truth that’s been present all along.
Excerpt from Chapter 1:
He stumbled as he was yanked backwards by the chain of the handcuffs, Mr. Morrison’s cruel hand having snaked under his Belstaff to assert his control. John met his eye. He nodded once, almost harshly, in the regimented yet reassuring manner of a soldier. Sherlock knew he was painfully squeezing his hands together behind his back as if to punish himself; lips bitten red and a tightly clenched jaw also betrayed the worry behind his steely façade.
“I’ll kill him,” John said, terrifyingly calm. “I’ll kill him and bring you home. I will.”

Thanks to Sherlock's inability to resist satisfying his curiosity, he's been turned into a small child of about seven (or five, depending on who you're asking) years old. Rather than let Sherlock be taken to a laboratory, John volunteers to look after him. After all, he's had to look after a man-child, how hard can a child-child be?
Poor John. He has no idea.

Summary
After not hearing from John for months after his daughter was born, Sherlock starts receiving hand-written letters from him telling Sherlock about his day, asking him how he's been doing, asking him for help...it's the last bit that gets them both in to a bit of a bind.
"John likes to start his letters with “Dear Friend”, but Sherlock can’t bring himself to use the term in return. It feels like John has been his only true friend throughout his entire life, and John knows that already.
And if John will not allow him to sign his letters, he decides that he will end them all the same from here on out.
When you’re ready to stop being an idiot…
When you’re ready to stop fighting this stupid battle of who could care less…
When you’re ready to let me help you…
When you’re ready to be done sneaking around…
When you’re ready to speak to me properly again…
When you’re ready to come home…He says all of these and more with three simple words: “When you’re ready…” that, in the end, roughly translate to “I’m here. I’m waiting. Come home.”"
