“How am I here?” Regulus asked breathlessly, “I shouldn’t-”
“Kreacher.” Sirius said, “He found me and took me to you.”
Regulus's eyebrows furrowed, “That’s impossible.”
Sirius shrugged, and crossed his arms over his chest with a scowl, “It’s what happened.”
“I told him he wasn’t to tell any family-” Regulus started when realisation dawned on both their faces,
“I’m no longer a part of the noble and most ancient house of Black, Reg,” Sirius said, he had an odd expression on his face like he was surprised Regulus could have forgotten.
Kreacher finds a loophole that saves Regulus's life— and now everyone has to deal with the consequences.

Regulus hadn't exactly meant to lie low amongst muggles — when he'd grabbed the Dark Lord's Horcrux he'd meant to destroy it on the spot. When he hadn't been able to, he'd meant to send Kreacher to destroy it. When Kreacher had refused that and dragged them both out of the cave…
Faking his death and going on the run until he could work out how to destroy the Horcrux had been the only real option.
